Caretrace Privacy Policy

This privacy policy ("policy") describes the information Caretrace (“we”) collects when you visit and use our website (“the site”, www.caretrace.com), as well as how we use the data to support our features and services.

By continuing to use the site, you acknowledge that you have reviewed this privacy policy, agree to its terms and you have consented to the use of your information such as personal data and device information, and the applicable disclosures.

1. THE DATA WE COLLECT

Generally, we do not require any personal data when you visit our website only without using any of our services. When you decide to use our services or features, you are required to create and account and we collect various information to assist in providing you with our services and improve on existing features. You will be required to provide the details such as:

  • Email address
  • First and last names
  • Age
  • Gender
  • Measurement of patient waist
  • Patient weight and height
  • Patient diabetes type
  • Patients’ blood sugar data
  • Patient treatment and medication information
  • Patients diet data (food log, calorie count, water intake)
  • Patients fitness data (daily steps, other exercises)

We also collect certain information about your device when you visit the website, including information about your web browser, IP address, time zone, location and some of the cookies that are installed on your device. 

2. WHY WE COLLECT YOUR DATA?

 We intend to use the information collected for the following purposes:

  • For generating specific meal, fitness, medication and treatment plans for each patient
  • For determining their risk exposure of becoming diabetic
  • For classifying them into groups for what type of care they should receive and how urgently or specifically that care should be administered

3. WE USE THE FOLLOWING TECHNOLOGIES TO COLLECT YOUR DATA

We use methods that ensure secure and efficient data transmission and processing. We use cookies for data collection. The type of cookies we would use are: 

  • Track patient activity on our website
  • Remember responses and store information on incomplete forms

We also collect data using the following technologies:

  • Through sign up pages on mobile app
  • Through patient profile update page on mobile app
  • Through diabetes risk assessment page on website

4. HOW WE STORE YOUR DATA

The information collected is stored securely on AWS cloud, in the US. Should we need to change our cloud service providers in the future, all parties will be duly notified, and all data will be moved to new provider.

5.  SAFEGUARDING AND SECURING THE DATA

Your Personal Data shall be collected and processed in accordance with specific, legitimate and lawful purpose consented to by you provided that a further processing may be done only for archiving, scientific research, historical research or statistical purposes for public interest.  

  • Installing  a security architecture on our servers to help protect user information
  • Having an in-house cyber security expert that monitors patient data and tracks irregular access to our database
  • Controlled access of critical data only to member of our team with security clearance and HIPAA compliance training. 
  • Follow NDPR and NDPC policy on data privacy
These measures collectively uphold our commitment to safeguarding user data and maintaining the integrity and confidentiality of our platform.

6. RIGHT TO RESTRICT THE COLLECTION OF YOUR PERSONAL DATA

At some point, you might wish to restrict the use and collection of your personal data. You can achieve this by doing the following:

When you are filling the forms on the website, make sure to check if there is a box which you can leave unchecked, if you don't want to disclose your personal information.

7. ACCESS TO YOUR DATA AND RIGHT TO MAKE CHANGES

You can access and update your information conveniently through our mobile application, ensuring transparency and control over their personal data. Here's a process of updating or making changes to your information:

•Updating Information: Users will be able to update their data through the profile page on the mobile app.

8. ACCESS, CONTROL AND STORAGE OF THE INFORMATION COLLECT

Who will have access to the information collected and who will control the use and storage of such information?

  • Our chief technology officer only will have access, control the use and storage of the information collected, with security clearance and HIPAA compliance and certification.
  • THIRD PARTY ACCESS TO DATA: We would not engage third-party service providers. All information will remain solely with CARETRACE. If a patient uses more than one solution from CARETRACE, e.g., mobile app and web app, information will be shared between both platforms, but not with a third party.

9. DATA RETENTION

Identifiable data such as name, email and date of birth will be stored for as long as the patient keeps their account with us. None identifiable data such as disease condition type, treatment or medication type, we will store all through the years as CARETRACE exist. Patients will be asked for consent for non-identifiable data to be used for research to make healthcare better, if they select yes, then the info will be kept all through the years as the company exists, and if they say no, all data will be deleted a patient deletes their profile

If a user deactivates their account, their data will continue to be stored for 10 years, after which, all information about the user will be removed completely from our servers. If the patients reactivates their account during the 10-year window, then their data will continue to be with CARETRACE.

10. REMEDY FOR VIOLATION OF THIS POLICY

We are committed to protecting the data you provide us, in line with this policy. However, in the unlikely event that we violate this privacy policy, you may contact us to notify us of the violation and request redress. 

When notice of a violation is investigated and confirmed, the act or omission causing such violation shall be immediately corrected and an amicable settlement reached to compensate you where necessary. 
All claims for remedy shall be resolved not later than 30 days after the complaint was lodged. 

11. NOTIFICATION OF CHANGE OF POLICY

Users will get series of email notification, followed by an in-app notification when there is a change in privacy policy.

12. LINKS TO OTHER WEBSITES

Our website contains links that lead to other websites. If you click on these links, Medvax Health is not held responsible for your data and privacy protection. Visiting those websites is not governed by this privacy policy agreement. Make sure to read the privacy policy documentation of the website you go to from our website.
